A phase diagram plots the stable phase at each temperature and pressure. Crossing a boundary line triggers melting, boiling, or sublimation. The three boundaries meet at the triple point, where solid, liquid, and gas coexist, and the liquid-gas line ends at the critical point beyond which the distinction disappears into a supercritical fluid.
